• The standard Order Form is for purchasing books not intended for resale. (PDF format)

• The Resale Order Form is for use by stores which plan to sell our books. (PDF format)
 

Ordering

• To order, mark a “1” in the “Item” column for the first item and the quantity of that item you are ordering in the “Quantity” column in the row for that book title.
• Multiply the List Price for that item by the quantity you are ordering and write that amount in the Item Total column in the same row.
• Repeat for any other items you are ordering.
• Add Item Totals and mark the sum in the Book Total box below all the Item Totals.
• Shipping & Handling via Media Mail for one or two books other than In Here in a padded mailer is $3.35. 3 or 4 books or In Here: $3.83. Via Priority Mail: $6.45 (i.e., up to 1 lb.).
   Shipping & Handling via Media Mail for one copy of In Here or for three or four other books is $3.83. Via Priority Mail: $9.60 (i.e., up to 2 lbs.).
    Via Priority Mail: up to 3 books or one copy of In Here: $6.45
   Shipping & Handling for the broadside via Media Mail is $3.35 and via Priority Mail is $6.45.
   Shipping & Handling for the set of 19 available books via Media Mail is $6.00 and via Priority Mail is $12.90.
• Enter the appropriate value in the Shipping & Handling Box.
• Add the Book Total to the Shipping & Handling and enter that for Balance Due and for Amount Enclosed.
• Make out and enclose a check for the Balance Due amount with check made out to Hummingbird Press and mail to
